Our SNAP Notice letter will give you an instructional guide to upload all forms.

Add noreply@studentehr.com to your email address book now to ensure proper delivery of any notifications.

To access your SNAP account for the first time (parents/guardians only):

  • Go to studentehr.com 
  • Enter your email address (same email used in blackbaud)
  • Click the “Forgot Password” link.  It will email you a link to set up a new password. 
  • Return to studentehr.com and log in using the new password.

Parents/guardians are encouraged to call the School Nurse any time they have questions, or concerns and specifically for the following reasons:

  • A new medical diagnosis, allergy, or a change in your child’s health status
  • A newly prescribed medication
  • A change in current medication
  • A serious injury, illness or hospitalization
  • A fracture, sprain, stitches or a cast
  • A contagious disease such as chicken pox, flu, strep throat, pneumonia, whooping cough
  • If your child is absent for an extended period of time
  • If there are any recent changes in your family that may affect your child such as birth, recent loss, military deployment, sudden illness

ANNUAL FORMS

(these forms need to be updated EVERY year):

Permission for Medical Treatment – (2021- 2022) – THIS FORM IS REQUIRED TO BE SUBMITTED EACH SCHOOL YEAR PRIOR TO THE FIRST DAY OF SCHOOL.This allows appropriate MacDuffie staff to act in loco parent is in the event your child needs medical attention while on campus.


MA School Heath Record – Forms that can be submitted in lieu of the MA School Health Form include a computer generated physician’s office form that includes the same information (must be in English language).
A school health record is considered current within 12 months of the signed date, and must be updated yearly. (must be in English language).
Any boarding student that does not have a current physical exam record will be sent to our school affiliated physician for an annual exam.


Certification of Immunization – Forms that can be submitted in lieu of the MacDuffie Vaccine form include a computer generated physician’s office form that includes the same information, or Titre testing result documentation. Immunization waivers may be accepted for medical or religious reasons only, and will follow the Reportable Disease and Isolation and Quarantine Requirements (105 CMR 300.00).


Vaccine Administration Consent for REQUIRED Vaccines (FOR INTERNATIONAL STUDENTS ONLY) –
If students have not received ALL state vaccines required for school attendance, they will be required to receive missing vaccinations here at school, or at our affiliated medical office, unless a medical or religious waiver is on file.

If boarding or home-stay students have not received ALL state required vaccines, boarding students will be required to receive missing vaccinations here at school, or at our affiliated medical office, unless a waiver is on file.


Prescribed Medication Authorization (FOR STUDENTS THAT WILL NEED PRESCRIBED MEDICATION WHILE ON CAMPUS) – All Prescribed Medication orders must be United States based. No medication orders from outside the USA will be accepted. Boarding students that need USA based medication orders will be sent to our affiliated physician and this form can be used as a suggestion for our physician to consider. Our physician has the right to not prescribe any suggested order. Please email the nurse if your child will need a medication appointment.


Over-the-Counter (OTC) Medication Authorization (2021 -2022) – We supply “OTC” medications for common ailments. Do NOT send any medication with your child (except rescue inhaler, epi-pen, and/or insulin), they will be confiscated per school policy.


Tuberculosis (TB) Screening (FOR INTERNATIONAL STUDENTS ONLY) – Students residing in TB “high burden” countries are required to obtain either a Tuberculin PPD skin test annually, or have a record of administration of BCG vaccine prior to school attendance. Health Insurance Disclaimer – Please read this document carefully. All students are required to have health insurance, and the health insurance information must be on file in case of an emergency.
International students will be enrolled in the minimum school health insurance plan (Bronze) with the option to upgrade, for an additional fee, to a Silver or Gold plan. Bronze plans WILL NOT cover the entirety of an emergency or routine medical bill. Upgraded plans cover more medical treatments, and require lower family out-of-pocket expenses. IT IS HIGHLY RECOMMENDED to upgrade. Please contact Ms. Shaw at bshaw@macduffie.org for pricing or to upgrade your insurance plan.


Optional Consent Forms

(these are not required, but offered if you choose)

Flu Consent (2021-2022) – THIS VACCINE IS OFFERED TO ALL STUDENTS, and given free of charge to students under 19 years old under the MA vaccine for kids state. Submit this form with the “consent” line initial AND parent signature. If all areas are not completed, the flu vaccine will not be given.


Human Papillomavirus (HPV) Vaccine Consent – THIS VACCINE IS OFFERED TO BOARDING STUDENTS ONLY, and given free of charge to students under 19 years old under the MA vaccine for kids state.

This vaccine is given in multiple dose. All doses will be given per CDC scheduled requirements. Seniors must start the vaccine series with enough time to complete the series while still on campus.
